The proof approval is the single most valuable piece of paper in a sign shop: it's the moment responsibility for the content moves from you to the customer. A signature alone doesn't do that well — customers sign without reading. A checklist does: making them tick 'all spelling and phone numbers are correct' line by line is what actually gets the artwork read.
This form includes the two clauses that end arguments before they start: colours approved as representative (screens are not printers), and the plain statement that production builds exactly this proof — approved errors are the customer's reprint. Blunt on paper, friendly in person; that's the right order.

Why use a checklist instead of just a signature line?
Because signatures get given without reading, and checklists get read. Ticking 'all spelling is correct' line by line is friction on purpose — it's the moment the customer actually proofreads, which is the whole defence against the $800 reprint with the wrong phone number.
Are 'colours approved as representative' clauses enforceable?
They set expectations more than they litigate: screens show RGB, printers lay CMYK, and the clause makes that conversation happen before production instead of after. For colour-critical brand work, the answer is a printed sample — and the form leaves room to note one.
What if the customer approves verbally or by text?
Then you have an argument waiting to happen. 'Looks good 👍' doesn't say which version, at what size, with which colours. A dated form (or a digital approval link) attached to a specific proof version is the standard worth holding.
